Also called pompholyx or vesicular eczema. Dyshidrotic eczema is a form of eczema often seen on the hands and feet where tiny blisters of serum form just below the skin's surface.

Dyshidrotic eczema is a recurrent disease of the palmar and or plantar skin characterised by sudden eruptions of clear, mostly pruritic vesicles. Dyshidrotic eczema may occur in both men and women.

Its onset occurs usually before the age of 40. Individuals experiencing emotional stress may be more likely to develop flare-ups of this disease.

The term ‘eczema’ is derived from a Greek word meaning ‘to boil.’ It refers to an inflammation of the skin which results in the formation of vesicls or pulstules. It is the most common and most troublesome of all skin diseases.

Eczema is essentially a constitutional disease, resulting from a toxic condition of the system. The disease covers a wide variety of forms, the majority of them being of a chronic variety.

Signs & Symptoms

Eczema in its acute form is indicated by redness and swelling of the skin, the formation of minute vesicles and severe heat. If the vesicles rupture, a raw, moist surface is formed. From this, a colourless discharge oozes, which forms skin crusts when it accumulates.

The disease is usually worst at night when the heat of the body is retained by the bed-clothes. The skin itches at all stages. IN the wet stage, it may become infected with bacteria. The healing of the condition is affected by scratching in response tothe irritation. Scratching not only spreads infection but also lengthens the stage of dryness and scaling.

Eczema appears as elevated, very parched, crusty, usually discolored patches on the skin. It is a puzzling skin ailment, caused by stress, diet, allergies, or from using highly chemicalized body care products, including dish detergents.

Common symptoms of Dyshidrotic Eczema are :-

• Intense itching at the site of the blistering.
• Pain may occur with larger blisters.
• Cracks or fissures on the fingers or toes.

Types of Dyshidrotic eczema

What Causes Dyshidrotic eczema?

The exact cause of Dyshidrotic Eczema is not known. Some consider it is caused by abnormal sweating. The condition may be mild with only a little peeling, or very severe with big blisters and cracks which prevent work.

The first (acute) stage shows tiny blisters (vesicles) deep in the skin, associated with itching and a burning feeling. The later and more chronic stage shows more peeling, cracking, or crusting.

Some patients will have mostly one stage, and some patients will have mostly the other. Some times both stages occur at the same time.

Secondary infection with staphylococcal bacteria is not infrequent. The result is pain, redness, swelling and crusting or pustules.

Allergies play an important role in causing eczema in children. It is seen that the disease is more frequent in bottle fed baby than breast fed baby. It has therefore been suggested that cow's milk could be the cause for infection in a particular baby.

According to ayurveda, the cause of eczema is the accumulation of toxins in the body, and their inadequate elimination, leads to eczema.

Allergies play an important part in causing eczema. Some women get eczema on their hands due to an allergy to soap or detergents used to wash clothes or dishes. Some persons develop it around the fingers when they wear rings because of allergy to metals.

Researchers at the University of Texas Health Science Centre at San Antonio, in a recent study of children with atropic eczema, found that 75 per cent were allergic to a number of foods. The most common triggers for sensitive persons are eggs, peanuts, chocolate, wheat, cow’s milk, chicken and potato.

The real cause of eczema however is the failure of the human system to excrete the poisons from the various orifices of the body. Waste matter is excreted from the rectum through stools, from the bladder through urine, from the lungs through breath and from the pores of the skin through sweat.

Sometime the pores of the skin are overworked as waste matter is not properly eliminated from the other orifices. If the pores are not given the chance to perform their normal function, the sweat will be full of morbid matter and this gives rise to skin diseases like eczema, acne, boils and other eruptions.

Other causes include faulty metabolism, constipation, nutritional deficiencies and stress brought about by nagging spouses, jealousy, frustration and a host of other emotions. Suppressive drug treatment of the formal disease is also a most potent subsidiary causative factor in many cases.

Who is at risk?

First, and most obvious, stop scratching. This may seem very difficult, but when you stop, the rash goes away. Remember that scratching your skin could cause outbreaks again in the future, so find clothing and towels that are soft to the touch.

Also avoid soaps and shampoos that are harsh on your skin and try to refrain from frequent hand washing, exercise, or extreme temperatures. Also test your skin. You may be allergic to something in your environment that triggers eczema.

Your doctor can help you find creams and other topical therapies to help you manage the itchy skin when your rash breaks out. You can also learn about prescription medications to help with your condition. With a little research, you can treat the eczema so that it doesn’t control your life.

Eczema in Children:

When a skin rash cannot be blamed on poison ivy or chicken pox, the culprit is eczema. In this condition, the skin turns red, scaly and develops sores. The itching is beyond the limit and the skins starts to shed in the form of scales. Dermatitis is the other name for eczema.

Dermatitis actually means inflammation of the skin and the skin turns pink and sore. Eczema is a common child problem as out of ten kids at least one kid gets eczema and majority of them get it before they turn five years old. Children who are above five years are also at a risk of developing eczema, but once they become teenagers, the chances are negligible.

Not only does eczema dries the skin, it also makes it itch horribly. The skin break out can even take the form of rashes. Eczema is a condition which comes and goes frequently and therefore it is chronic. When any foreign bodies come in contact with the skin, a special kind of cells present in the skin will react to them.

Basically, they inflame the skin as a measure of protection. The harmful foreign objects trigger the reaction and make these special cells to over react. This turns the skin itchy, sore and red. Children, who get eczema frequently, have a larger quantity of these special cells.

Usually, people who have family members with asthma, hay fever or any allergies are more prone to getting eczema. It is in their genes to get eczema and this problem is passed over to them by their parent. Some scientists say that people, who as children have had eczema, are at a higher risk of developing asthma or hay fever later in their life. Allergies can only make eczema worse. The good news is that eczema is not contagious, so children can be happy about not catching the infection.

The first symptom of eczema is rash development. These rashes may seem to go away in the beginning, but will return back. Although eczema itches badly, not all rashes will itch. Eczema will start to develop behind the knees and inside of the elbows. It will slowly spread to the face and other body parts.

Because rashes can be caused due to many other reasons apart from eczema, the doctor will examine the rashes and its cause very carefully. If eczema is confirmed, the child may have to use a moisturizing cream or lotion to stop the itchiness and drying of the skin. In serious cases, the patient will be recommended corticosteroids. It is a steroid cream or ointment which needs to applied to check skin inflammation.

If the itching is really irritating, antihistamine can be taken in either liquid or pill form. And if the scratching had resulted in an infection, antibiotics will also be prescribed. The problem doesn’t get solved forever, but it makes the condition better for the time being.

There are certain things which trigger eczema and should be avoided by children who get eczema frequently. Perfumes, detergents, soap, dry air in winters with very little moisture, and sweaty and hot skin. Also when coming in contact with dust mites and scratchy fabrics can increase skin irritation. The child must abstain from scratching even if the skin is itching badly, as that will make the skin itch more badly.

The skin can also get infected with bacteria because of skin breaking and bleeding. A washcloth can be dipped on cool water and placed on the area of itchiness. The parents should cut the nails short of the child so that he/she doesn’t hurt himself while scratching. Lastly, lots of water should be consumed to provide moisture to the dry skin.

Treatment/Therapy:

Eczema is a dry skin condition that affects many adults and children. It occurs in patches that can become thick and red, and it can often occur in those with allergies such as hay fever. Topical creams can be applied to help the skin recover.

Some of common treatment of Dyshidrotic Eczema are :-

• Soaks or compresses using weak solutions of Condy's crystals (potassium permanganate), aluminium acetate, or vinegar in water, are applied for 15 minutes four times a day. This will dry up blisters. Compresses are not suitable for dry eczema.

• Moisturizers with cream.

• Corticosteroid creams and ointments play an important role in the treatment of Dyshidrotic Eczema.

• Oral anti-pruritics such as Atarax or Benadryl may alleviate itching

• Antibiotics such as flucloxacillin.

• PUVA therapy.

Expectations (prognosis)

Dyshidrotic eczema normally resolves without problems. Uninhibited scratching and skin trauma may lead to thick, irritated skin which is more difficult to treat and has a longer healing time.

Skin applications to cure eczema may give temporary relief, but if the exudation is suppressed, some other more serious disease of childhood may develop. The best way to deal with eczema is to cleanse the blood stream and the body.

• When there is a fresh patch of dry eczema, applying freshly made ghee (from cow's milk butter only) and washing the area well a number of times, relieves the itch.

• Dry the peel of a watermelon, the large variety that has a deep green skin and bright red pulp with black seeds. Burn the dried peel to an ash. Take a spoonful of this ash and add enough warm coconut oil to make a paste. Apply this on the eczematous patch.

• Use a diluted neem decoction for washing the weepy lesions twice a day then pat them dry.

• Apply a paste made with a tablespoon of fresh, crushed neem leaf and a teaspoon of turmeric powder. If possible, use a fresh turmeric root. Add a spoonful of sesame seed oil. Apply once a day.

• Boil a tablespoon each of mango tree bark and acacia tree bark in half a litre of water. Filter and use this to wash the eczematous patches. The water should be warm enough to act as a fomenting agent, but not hot enough to burn. Pat dry and apply fresh ghee.

• Use of black strap molasses (sheera) has been found beneficial in this treatment of this disease. This is presumably due to its high nutritive properties. One tablespoon of molasses mixed in a half a glass of milk should be taken twice daily by grown up children. Improvements will be noticeable within two weeks time.

• Coconut oil may be applied to the portions with eczema. It will help the skin to stay soft. Sunbathing is also beneficial as it kills the harmful bacteria and should be resorted to early in the morning, in the first light of dawn.

The baby or child should get as much fresh air as possible. He should drink plenty of water and bath twice daily. The skin with the exception of part affected with eczema, should be vigorously rubbed with the palms before taking bath. The child patient should avoid tea, coffee and all condiments and highly flavored and tinned or bottled foods.

The treatment should start with a fast on orange juice and water from five to days, depending on the severity and duration of the trouble. Juice fasting will help eliminate toxic waste from the body and lead to substantial improvement. In some cases, the condition may worsen in the beginning of the fast due to the increased elimination of waste matter through the skin. But as fasting continues, improvement will manifest itself.

Fruits, salt free, raw or steamed vegetables with whole meal bread or chappatis may be taken after the juice fast. Carrot and musk melon are particularly beneficial. Coconut oil may be used instead of ghee. After a few days, curd and milk may be added to the diet. The patients may thereafter gradually embark upon a well-balanced diet of three basic food groups, namely (i) seeds, nutsand grains (ii) vegetables and (iii) fruits. The large proportion of the diet should consist of raw foods. Seeds and beans such as alfalfa, mung and soyabeans can be sprouted.

This diet may be supplemented with cold-pressed vegetable oils, honey and yeast. Juice fasting may be repeated at intervals of two months or so, depending on the progress being made, in chronic and more difficult cases of eczema, patient should fast atleast once a week till he is cured.

The patient should avoid tea, coffee, alcoholic beverages and all condiments and highly flavoured dishes. He should also avoid sugar, white flour products, denatured cereals like polished rice, and pearled barley and tinned or bottled foods. He should eat only pure and wholesome foods. Raw vegetable juices, especially carrot juice in combination with spinach juice, have proved highly beneficial in the treatment of eczema. The formula proportions considered helpful in this combination are carrot 300 ml. and spinach 200 ml. to make 500 ml. or half a litre of juice.

The patient should get as much fresh air as possible. Restrictive clothing should not be worn. Two or three litres of water should be taken daily and the patient must bath twice or thrice a day. The skin, with the exception of the parts affected with eczema, should be vigorously rubbed with the palms of the hands before taking the bath.

Coconut oil may be applied to the portions with eczema. It will help the skin to stay soft. Walking or jogging should be resorted to in order to inactivate the bowels. Sun bathing is also beneficial as it kills the harmful bacteria and should be resorted to early in the morning, in the first light of dawn. A light mudpack should be applied over the sites of the eczema is also helpful. The pack should be applied for an hour at a time and should be repeated twice or thrice a day .

Self Care strategies for Living with Dyshidrotic eczema

Eczema can flare up when you are under stress. Learn how to recognize and cope with stress. Stress reduction techniques can help. Changing your activities to reduce daily stress can also be helpful.

Cut down on all non-vegetable proteins, sugars, oils, white flours products, and dairy products

Try not to scratch the irritated area on your skin even if it itches. Scratching can break the skin. Bacteria can enter these breaks and cause infection.

I have found that drinking tomato juice (not the cocktail) clears up the eczema in a matter of days. If you need to, add some pepper or your favorite spice to give the juice flavor.

Apply aloe vera and take evening primrose capsules and also apply topically

Moisturizers help keep your skin soft and flexible. They prevent skin cracks. A plain moisturizer is best. Avoid moisturizers with fragrances (perfume) and a lot of extra ingredients. A good, cheap moisturizer is plain petroleum jelly (such as Vaseline). Use moisturizers that are more greasy than creamy, because creams usually have more preservatives in them.

Make the green tea as normal, but make it strong. Add 3-Tbsp Of Lemon Juice, 1-Tsp ground Rosemary, One Tsp Olive Oil to One Cup of Green Tea. Let this cool. (not cold, just room temp) Bath skin with this. (Recommended 100% Cotton Wash Cloth)

Examine your bathing rituals. If you suffer from eczema you should avoid taking baths daily with typical soaps and cleansers. Theses soaps can be irritating to your skin and have harsh effects. It is important to bathe with hypoallergenic soap products that are color and fragrant free. It is vital to find cleansers that will moisturize and hydrate your skin.

Provide further home eczema relief by using strong moisturizers such as Eucerin lotion. This will help to seal in moisture which diminishes redness and helps against itching. You can stock up on Eucerin at your local pharmacy or grocery store.

Water Treatment

In cases of acute eczema, cold compress or cold wet fomentations are beneficial. The affected part should be wrapped with a thick soft cloth.

The cloth should be moistened with cold water (55 o - 60 o F) every 15 to 30 minutes for two hours at a time. The bandage should be left intact, keeping the cloth cold.

There may be intensification of itching or pain initially but this will soon subside. A cold compress may be applied twice daily for a week or so.
